1 - Start a new game on any episode

2 - Type "engage61", "engage62" or "engage63"
to play the 3 super secret levels. I think they have no exit.

These are indeed the deathmatch levels; they were E4 in the original (three-episode) Heretic, and the first two *do* have exits to the next, it's just that (these being deathmatch levels) the exit is a lot harder than usual to find.
